If you are not able to keep your appointment, please cancel your appointment promptly so that other patients can be accommodated. On visits when tests are ordered (Lab, x-ray, Ultrasound, MRI, etc) by a physician in our office, you MUST schedule a follow up appointment to discuss the test results. Also, it is your responsibility to pay your co-pays at the time of your revisit by cash, check, or credit [Discover not accepted]. For your convenience, our receptionist will schedule your follow-up visit before you leave the office that day. We appreciate your understanding. There will be a $5 surcharge for all co-pays not paid at time of visit and there will be a $25 missed appointment fee for any appointment not canceled. Both cash and check are accepted. There is also a $25 surcharge if the check bounces.
